Job Title Lead Care Manager Status Full-Time, Hourly Reports To CalAIM Supervisor

Overview The Lead Care Manager position at Visalia Rescue Mission provides family-centered, member-focused, relationship-based care coordination services to individuals eligible for and enrolled in Enhanced Care Management services. This position effectively utilizes Agency policies and procedures and makes appropriate independent judgments in the course of daily work, especially in consideration of Visalia Rescue Mission population of focus

Essential Functions Responsibilities • Supports and promotes the mission of the Agency. • Responsible for coordinating with the member’s service providers and support network to ensure a seamless experience for the member and non-duplication of services. • Actively engage ECM-eligible members. • Oversee the provision of ECM services and implementation of the care plan. • Offer services where the member lives, seeks care, or finds most easily accessible and within managed care plan guidelines. • Connect ECM members to other social services and supports they may need, including transportation. • Advocate on behalf of ECM members with health care professionals. • Utilize motivational interviewing, trauma informed care, and harm reduction approaches. • Coordinate with hospital staff on ECM member discharge plans. • Accompany ECM members to office visits as needed and according to managed care plan guidelines. • Monitor treatment adherence, including medication. • Provide health promotion and self-management training. • Carries a full caseload of 50 ECM members, unless instructed otherwise by senior management within the scope of policy guidelines. • Identifies gaps in and barriers to service, documenting in the member’s care plan, and communicating these to the multidisciplinary care team to develop and implement a response. • Prepares for and participates in individual and group supervision meetings
Behaves in an ethical, professional manner in support of Agency culture. • Identifies and presents discussion topics with management staff regarding potential workplace and morale issues, making recommendations for addressing them. • Displays positive and attentive attitude at site meetings, as well as department, program and Agency staff meetings. • Works effectively with the program and site coordinators and the department head, accepting direction and implementing policies and procedures reflective of this direction. • Displays sensitivity to the service population’s cultural and socioeconomics characteristics. • Always performs work in a safe manner and ensures that other individuals do not put themselves or others at risk. • Follows all policies, procedures and legal mandates including HIPAA, member confidentiality, releases of information, protected health information (PHI) and member files. • Ensures performance and behaviors support the Agency’s performance quality improvement (PQI) goals and objectives. • Reports to work on time and maintains reliable and regular attendance. • Performs other duties as assigned. • Maintains positive, cooperative, life-giving team dynamics and environment. • Attends training and team meetings. • Other duties and responsibilities as required by the Executive Director(s), Program Director(s), or Supervisor. • Fulfill all aspect of the job description according to the biblical principles and philosophy of ministry as defined by the Visalia Rescue Mission Statement of Faith, Vision, Mission Statement and the Core Values.

Qualifications, Knowledge, Critical Skills, Abilities • Active understanding of and participation in supporting VRM's mission, goals, and values. • Strong communication skills: The ability to listen, empathize, and communicate effectively is essential. • Advocacy skills: The ability to advocate for clients' rights and needs is important. • Organizational skills: The ability to manage time, prioritize tasks, and stay organized is necessary. • Ability to demonstrate a professional presence and appearance. • Actively involved in the local church community. • High school diploma or equivalent. • Familiarity with DHCS/CalAIM guidelines and policies, including Medi-Cal, care coordination models, healthcare best practices, and cultural sensitivity. • Strong interpersonal and communication skills on English (at minimum) for effective collaboration with population of focus, healthcare providers, and community organizations. • Proficiency in care assessment, planning, and implementation processes. • Ability to utilize technology and data management systems effectively for case documentation and tracking. • Willingness to foster partnerships with local agencies, institutions, or community organizations to enhance resource availability and support Medi-Cal members. • Capacity to administer regular internal audits of billing records to identify discrepancies and areas for improvement • Education or background in social work, nursing, psychology, public health, or a related field. • Experience in fields such as care coordination, case management, working with children and youth, or related fields. Environmental Conditions • Indoors in a typical office environment with little exposure to excessive noise, dust, fumes, vibrations, and temperature changes approximately ninety-five percent (95%) of the time. • Outdoors with exposure to noise, heat, and cold approximately five (5%) of the time. • Frequent use of the computer at a workstation, up to eight hours per day. • May occasionally move (walk or drive) from one work location to another. Physical Demands • Mobility within the office and ability to talk, hear and use hands and fingers to handle office equipment, records, and files. • Vision abilities required include the ability to read paper and electronic documents. • Valid CA Driver’s License.

Compensation • Pay based on experience within a designated range. • Health benefits the first of the month following sixty (60) days of continuous employment.
